Rochester Regional Health recently opened its first ElderONE facility in Wayne County.
ElderONE Silver Hill, 1000 Technology Parkway, Newark, makes RRH’s Programs of All-inclusive Care for the Elderly model of care available to seniors in Wayne and Ontario counties.
At a PACE center, people ages 55 and older who are eligible for Medicare, Medicaid or both can receive coordinated care, services and socialization opportunities through one place while continuing to live in their own homes without sacrificing personal independence.
“There is a significant need for medical care in rural communities, especially for senior citizens,” said Eric Bieber, RRH president and CEO. “We believe you shouldn’t have to go far to get the care you need. That’s why we’re bringing important health care services out to the communities we serve. Opening this PACE center in Newark is a game changer for seniors who live in the area. More people are going to be able to live at home, stay healthier and enjoy a better quality of life.”
ElderONE Silver Hill is open to Wayne County and Ontario County residents. In addition to basic medical care, ElderONE provides home health care, social services, prescriptions, nursing care, rehabilitation, laboratory, X-rays, transportation to and from the facility and activity programs.
“As seniors age, their needs for medical and day to day assistance change,” said Kathy McGuire, senior vice president of behavioral health, home care and managed long-term care/PACE. “For those ages 55 and older with functional and medical needs, ElderONE can coordinate the care and services to address those needs in a simple and affordable way.
